The North Hamilton Community Health Centre (with funding by HNHB LHIN) is able to expand the “Feet First- Steps for Health Program” through partnership with the Haldimand-Norfolk Diabetic Education Centre and Grand River CHC. The program is now available to residents of Brant and Haldimand-Norfolk at several sites within this geographical area. Care is provided at GRCHC, and the three hospital sites in Simcoe, Hagersville and Dunnville.
Feet First – Steps for Health offers no-cost foot care to persons with diabetes who have a moderate to high risk of developing lower limb complications and have faced barriers accessing these services.
Services are provided by Jasmine Kalra (Chiropodist), Cristina Arandia and Melony Cooper, Registered Practical Nurses with Advanced foot Care Certification. Services include individual foot assessment, follow-up and treatment. Group educational sessions promoting foot care and self management will be provided.
Individuals may be referred by their health care provider or they may self-refer by calling the Grand River Community Health Centre (519) 754-0777 to make an appointment. Attendance at this program does not cause billing negations for the referring health care provider.
