About Grand River Community Health Centre

Primary health care as defined by the World Health Organization (WHO) is essential health care, made accessible to individuals and families in the community by means accessible to them.

The Ottawa Charter for Health Promotion echoes the sentiments of the WHO.  It states that the role of the health care sector must move in a health promotion direction.  A community health centre moves beyond its responsibility for providing only clinical services.  Grand River Community Health Centre promotes an integrated model of care that is sensitive and respectful.

Grand River Community Health Centre's journey began in 2005.  The provincial government announced funding for 22 new Community Health Centres in Ontario.  Key champions planned for a community health centre for Brantford and Brant County.  A new Community Health Centre became a reality in 2009 in downtown Brantford at 363 Colborne Street.

Grand River Community Health Centre celebrated its 10th anniversary in 2019.  The Centre continues to provide community based primary health care and offers a range of services to support the emotional, physical and social needs of individuals and communities.  The Centre opens channels between the health sector and the broader community.  The Centre creates opportunities to work together to achieve equity, a sense of belonging, health and wellbeing for all.  

Grand River Community Health Centre is proud to serve Brantford and the County of Brant since the beginning vision in 2005!
