Ontario Health has introduced Interprofessional Primary Care Teams (IPCT) to connect more people to primary care.  This clinic will help support newcomers to Canada and work toward providing them health care until they can transition to a permanent primary health care provider. 

 About the Program
The program is designed to get the immediate health needs of newcomers met over time.  The goal of the program is to improve health outcomes as we move toward a smooth transition from us to a permanent health care provider in the community.
 About the Interprofessional Primary Care Team (IPCT) 
This team will work together to provide a range of services to clients who do not have primary health care. 

The team is comprised of: 

Medical Doctor 

Nurse Practitioners 

Registered Nurses 

 Who is eligible? 
  • Must have been in Canada for less than two (2) years.
  • Do not have a primary care provider (i.e., doctor or nurse practitioner).  

*Exclusions: international students.

For those who do not have a provider, we are accepting new patients! Come by our Outreach Clinic to register or fill in the form below and drop it off with our reception team.
