Ontario Health has introduced Interprofessional Primary Care Teams (IPCT) to connect more people to primary care. We are providing chronic disease education to compliment the care you are receiving from your primary care provider.

About the Interprofessional Primary Care Team (IPCT) |
This team will work together to provide a range of services to clients who do not have primary health care or whose primary care provider does not have services to support their clients.
The team is comprised of: Registered Dietitian Registered Social Worker Registered Nurse Community Navigator/Social Prescriber |
Who is eligible? |
Everyone is welcome, whether you have a primary care provider (i.e., doctor or nurse practitioner) or not.
*The program is not intended for people that have extended health benefits or have third party coverage. |

Virtual Urgent Care |
Virtual urgent care might be appropriate for you (or a family member) if you have a medical issue that is NOT life-threatening but requires urgent medical attention and you're unable to access timely care through a primary care provider. In the event of a life-threatening emergency, please call 911 or go to your nearest emergency department. |
