The Memory Clinic operates in collaboration with the Brant Community Health System and the Alzheimer's Society of Brant.  The Memory Clinic provides support to your primary care provider to diagnose and treat memory difficulties.  

 About the program
  • There is no cost
  • Clinics take place once per week on Wednesdays
  • Appointments are approximately 2 hours
  • Access to our Memory Clinic team (Physician, Nurses and Alzheimer Society Counsellors)
  • Comprehensive cognitive testing to assist in diagnosing memory concerns
  • Family member(s) or a support person can attend with you
 Eligible clients
  • Must be 65+ years of age
  • Must be from Brantford or Brant County

  • Speak to your Primary Care Provider to complete the referral
