The Seniors Assessment Service (SAS) encompasses the Memory Clinic and Geriatric Clinic, and operates in collaboration with the Brant Community Healthcare System and the Alzheimer's Society of Brant.

SAS focuses on diagnosis and management of complex conditions in the elderly (including memory, mood, medication, and frailty).

About the program

A referral to SAS could include either Memory Clinic visit with Dr. A. George or Geriatric Clinic visit with Dr. M. Nicula, Dr. C. Campbell. Referrals are triaged based on issues identified by Primary Care Provider. A home visit may be offered if applicable.

Assessments generally take 2 hours and will include cognitive testing, assessment by nursing and/or allied health support, physical exam, medication review, and referral to services based on issues identified during visit.

Eligible clients

  • Must have a Primary Care Provider to follow up on recommendations made by the team
  • Generally 65+ years old age, younger patients will be assessed on a case-by-case basis
  • From Brantford or County of Brant

Where to park:

We're located East of the downtown core in Brantford (East of the corner of Colborne St. and Alfred St.). You can park in the lot at the corner of Alfred Street and East Avenue or Alfred Street and Colborne Street. Street parking is also available in the area as well as disabled parking spots in the rear parking lot.
