Outreach Services

Grand River Community Health Centre offers mobile outreach health clinics in the community. The outreach team offers health clinics in many different locations within Brantford and Brant County. 

GRCHC has a mobile medical clinic vehicle equipped to provide primary health care assessments, treatment, referral, supportive counselling, linking with community resources and advocacy!

Shelter Health

This program is focused on providing primary care for those who are homeless or at risk of homelessness.  A Nurse Practitioner and Registered Nurse work as a team to provide a full range of primary care services including:  physical and mental health assessment, management of chronic conditions, referral to specialists, connection with community resources, wound care, and other services. We work with clients to provide ongoing primary care. Clients do not need to have a health card to receive service.

We are accepting clients for referral to our Shelter Health program. Clients must be experiencing homelessness/ precariously housed, with no family doctor. Please see the referral form below:

Brantford Downtown Outreach Team (BDOT)

GRCHC partners with St. Leonard’s Community Services to staff this outreach team. BDOT is funded through the Health Canada, Substance Use and Addictions Program to provide outreach supports and harm reduction within the Brantford downtown community.

Ontario Telemedicine Network (OTN)

A Nurse Practitioner works with local counselling partners to support access to medical assessment and psychiatry consultation. The OTN program offers accessible specialist consults through videoconferencing on site at GRCHC.  OTN consults are only available with a referral from a health provider.
