Grand River Community Health Centre offers outreach health clinics in the community in many different locations within Brantford. Our team provides acute episodic physical, mental health and addictions assessments, treatment, referrals, linkage with community resources, and advocacy!
Shelter Health
This program is focused on providing acute, episodic care for those who are homeless or at risk of homelessness and without a family doctor. A Nurse Practitioner attends shelters within Brantford to provide: acute episodic physical, mental health, and addictions care with treatment, connection to community resources, wound care, and other services. We work with clients to connect to ongoing primary care. Clients do not need to have a health card to receive service.
Referrals to this service are made upon discharge from the local emergency department or by connecting with staff at the local shelter you are residing at.
