Caring for My COPD is a 10 week community-based pulmonary rehab program that aims to help people better manage their COPD and to reduce emergency room visits and hospitalizations for acute exacerbations ("flare ups").
The program consists of:
- Weekly education and exercise sessions for 10 weeks
- Follow up by a multidisciplinary team for 12 months
Our team consists of a Kinesiologist, Occupational Therapist, Social Worker, Dietitian, and smoking cessation support from a Registered Nurse.
Who is eligible?
Anyone diagnosed with mild to moderate COPD
Who is not eligible?
- Clients residing in long-term care facilities
- Clients who are medically unstable
- Clients with severe to end-stage COPD
Locations
The program is primarily offered in person at 363 Colborne Street in Brantford; however, program access can be tailored for clients from Brant, Haldimand, and Norfolk via virtual services and satellite locations.
