About the Program |
The Grand River Community Health Centre (GRCHC) is providing accessible, inclusive non-urgent health services through its Outreach Drop-in Clinic, available at two locations in Brantford and Simcoe. Brantford – St. Andrew's Church, 95 Darling Street

Clinic Care Teams & Services |
These Drop-In Clinics are designed to support individuals who do not currently have access to primary health care. Each clinic is staffed by a dedicated team of professionals committed to providing comprehensive, person-centered care. Outreach Teams:
Together, these teams deliver a wide range of services including medical assessments, mental health support, harm reduction, sexual health care, wound care, immunizations, and connections to community resources. |

Virtual Urgent Care |
