Ontario Health has introduced Interprofessional Primary Care Teams (IPCT) to connect more people to primary care.

 About the Program

Grand River Community Health Centre will be hosting the Outreach Drop-In Clinic at the 363 Colborne Street location in Brantford on: 

Tuesdays from 12:00 PM - 6:30 PM 

Thursdays from 1:00 PM - 3:00 PM 

Clients could receive: 

  • Wound care, harm reduction, cancer screening, immunizations, sexual health, mental health services, ID Clinic and more.
 About the Interprofessional Primary Care Team (IPCT)
This team will work together to provide a range of services to clients who do not have primary health care. 

The team is comprised of: 

Nurse Practitioners 

Registered Nurse

Community Navigator/Social Prescriber 

Outreach and Service Navigation Worker

 Who is eligible?
 This clinic will support individuals who: 
  • Use substances 
  • Have mental health issues 
  • Are unhoused or street involved 
  • Have difficulty making or keeping appointments 
  • Do not have a primary care provider and/or a health card

*Exclusions

  • Post secondary students and/or international students
  • People who have a primary care doctor or nurse practitioner
  • People who do not have barriers accessing primary care
