 About the Program

The Grand River Community Health Centre (GRCHC) is providing accessible, inclusive non-urgent health services through its Outreach Drop-in Clinic, available at two locations in Brantford and Simcoe.

Brantford – St. Andrew's Church, 95 Darling Street

  • Clinic Type: Outreach Drop-In Clinic (no appointment needed)
    Hours: Wednesdays, 11:30 AM – 3:00 PM

Simcoe – Norfolk Family Health Team (NFHT), 185 Robinson Street, Suite 101

  • Clinic Type: Outreach Drop-In Clinic (no appointment needed)
    Hours: Thursdays, 10:00 AM – 3:00 PM
Clinic Care Teams & Services

These Drop-In Clinics are designed to support individuals who do not currently have access to primary health care. Each clinic is staffed by a dedicated team of professionals committed to providing comprehensive, person-centered care.

 Outreach Teams:

  • Nurse Practitioner
  • Registered Nurse
  • Outreach and Service Navigation Worker

Together, these teams deliver a wide range of services including medical assessments, mental health support, harm reduction, sexual health care, wound care, immunizations, and connections to community resources.

 Who is eligible?
This clinic is designed to serve individuals who may face barriers accessing traditional health care. It is especially intended for those who:
  • Use substances
  • Are experiencing mental health challenges
  • Are unhoused or street-involved
  • Struggle to make or keep scheduled appointments
  • Do not have a primary care provider and/or lack a valid health card

Who Is Not Eligible

  • Are post-secondary or international students without local health care access
  • Have an existing primary care provider (doctor or nurse practitioner)
  • Do not face barriers accessing primary health care services
