Grand River Community Health Centre will be hosting the Outreach Drop-In Clinic at the 363 Colborne Street location in Brantford on: Tuesdays from 12:00 PM - 6:30 PM St. Andrew's Church at 95 Darling Street in Brantford will be hosting an Outreach Drop-In Clinic on: Wednesdays from 11:30 AM - 3:00 PM At the Norfolk Family Health Team at 185 Robinson Street, Suite 101 in Simcoe there will be an Outreach Drop-In Clinic on: Thursdays from 10:00 AM - 3:00 PM Clients could receive:

About the Interprofessional Primary Care Team (IPCT) |
This team will work together to provide a range of services to clients who do not have primary health care.
The team is comprised of: Nurse Practitioners Registered Nurse Community Navigator/Social Prescriber Outreach and Service Navigation Worker |
